THE FOLLOWING REQUIREMENTS FOR TESTING ARE NON-NEGOTIABLE.  ALL ITEMS MUST BE COMPLETED AND TURNED IN ON TIME.  THIS PAGE IS FOR INFORMATIONAL PURPOSES ONLY.  YOU MUST PRINT OUT THE ACTUAL FORM AND FILL IT OUT COMPLETELY.  ALL FORMS ARE ON THE MEMBERS ONLY TEST FORMS PAGE, WHICH YOU CAN FIND BELOW.  FOLLOW THE LINKS, PRINT OUT ALL FORMS, COMPLETE THE FORMS, AND TURN THEM IN ON TIME IN ORDER TO GET A PROMOTION.  FAILURE TO COMPLETE ANY OF THESE ITEMS WILL RESULT IN NO PROMOTION FOR YOURSELF OR YOUR CHILD.

ATTENDANCE- The students must average two classes per week between promotions.  If an employee does not attend work, they do not get raises and  promotions. This is recreational, not a job, but it is a life skill.

APPLICATION- Turned in FIRST week of the month. This allows us to order belts, have enough tape, and staff properly to accommodate the group testing.

FULL UNIFORM- Must be worn the weeks of testing and the day of promotion.  This is tradition; notice, if you will, all of our Brown Belt and above students wear a full uniform.  This is a physical cue to the Instructor that the student is prepared for their A-Game!

THE FIRST STRIPE/DEGREE- (For every belt) must update the contact information and new liability form for your student file, in case of an emergency.

GOALS DUE- The first week of the month of your scheduled exam.  The goals do not have to be Kung Fu San Soo related.  There will be THREE goals: 1) Short Term, to be accomplished within 3 months. 2) Medium Term, to be accomplished within a year. 3) Long Term, perhaps a five-year goal.  All goals need a start and end date. They must be personal and unique (not what a person feels obligated to say). If a goal has been established, an update on progress is expected.  If a goal changes, then share that.  ALL STUDENTS ARE REQUIRED TO DO THIS FROM YELLOW BELT AND BEYOND.

ESSAYS DUE- Yellow Belts and above on the Virtues of the Month. All students 21 and under, all classes.  Accountable for each virtue since the last test.  Due the first day of the first week of exam month. NO EXCEPTIONS.

NOTEBOOKS- Due at the end of class on Thursday and will be returned the following regular class. Adult and 6:00 class.

PARENT NOTE- must be signed and turned in the first day of testing week.  If the parent is working on a specific issue and is using the parent note or testing as a carrot (Motivator). Please let Master Jim know in advance or the child will not be eligible for the test.

TEACHER NOTE- must be signed and turned in the first day of testing week.  No Exceptions.  We are not training bullies. We are preparing future leaders!  Power, Honor, DISCIPLINE!  Teachers deserve a voice, and parents appreciate the opportunity to know of their child's behavior when they are not present. We encourage parents to be proud and praise their children for doing good and right things when they are not looking.

PRE-EXAM- Print your exam, do the skills, rate yourself on a scale of 1-10. A score of 1 is the lowest level of skill comprehension. A 10 is the highest level of comprehension.  Turn this in with your application please.

EXAM PRINTED- Name filled in and all information done.  This is your actual test!  Upon competition It will be put in your student file and returned to you when you achieve your BLACK BELT!

**IF you cannot do this outline and/or would prefer to take the exam privately or semi-privately that can be arranged:

White Belt through Yellow Belt THIRD STRIPE....$150.00

Green Belt Through Brown Belt THIRD STRIPE....$300.00

The promotions will be held the THIRD Saturday of the month, at PhD Martial Arts at 12 P.M. unless announced at the beginning of the month (3-weeks notice)
